Your first visit to our office

patient forms

Before your first checkup, you’ll need to complete our patient forms. This helps us understand your health history and any important information on your condition, which is essential for treatment.

For a faster appointment, you can download, print and fill out these forms before coming in. They are available in the New Patients tab.

The desk has informational pamphlets, and a sign with health-related information is visible next to the woman. There is a black TV mounted on the beige wall behind them.

Consultation

Once we’ve looked over your forms, we’ll review any health-related problems or concerns you have, and discuss potential treatment options.

Throughout the process, we learn more about you, your condition, and expectations so we can determine how chiropractic care can help you meet your goals.

Examination

Next, Dr. Owens will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as needed to ensure a full understanding of your condition.

Treatment plan recommendations

After the examination, Dr. Owens will start treatment. This may include spinal adjustments tailored to your needs, and myofascial release in key areas. Treatment is interactive, so you can express concerns about the different styles of treatment at any time.

Dr. Owens will also recommend a future care program of adjustments that allow for a thorough resolution of your problem.
